What the inspector wrote

On 10/1/2025 at 2:30pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Marcelina Badeo, Administrator and explained the reason for the visit. While LPA L. Hall was delivering findings for complaint 15-AS-20250805145625 on 10/1/2025. During record review LPA observed S2 was not associated to the facility. The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights provided
