What the inspector wrote

On 11/21/2023 at 9:00 AM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct an annual inspection. Upon arrival, LPA explained the purpose of the visit to Caregiver 1 (C1). Administrator (ADM) Administrator Rachelle Perez arrived at approximately 9:15 AM. During the Inspection, the LPA inspected the facility inside and outside. All outdoor and indoor passageways were free of obstruction. There were no bodies of water observed. Inside, the temperature was measured at a comfortable 68 degrees Fahrenheit. The LPA observed adequate lighting in all of the rooms for the comfort and safety of the residents. Residents’ bathrooms were observed to be equipped with grab bars and non-skid mats. LPA observed 7 days of nonperishable and 2 days of perishable foods on hand. Sharps and dangerous items were inaccessible to residents. Smoke and carbon monoxide detectors were in operating condition. Fire extinguisher was observed to be fully charged and serviced on 07/12/2023. LPA interviewed 1 resident, and reviewed facility, staff, and resident records. 4 Type-B citations issued (for details refer to LIC809-D). Annual inspection is incomplete. The LPA will return at a future date and time to complete the inspection. Exit interview conducted with ADM. A copy of this report provided via email.
