What the inspector wrote

On 3/4/202 at 2:00PM, Licensing Program Analyst G.(LPA) Carol Fowler arrived unannounced to conduct a case management visit to deliver amended report originally dated 01/28/2026. LPA met with Administrator, Benjie Doctolero and informed him the reason for visit. During visit, LPA attempted to obtained original report dated 1/28/2026. The facility had a change of Administrator, the current Administrator was not able to locate the report. No deficiencies are being cited on this date. Exit interview conducted. A copy of this report provided.
