On 04/22/2026 at 012:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required One Year inspection. Upon entry, the LPA stated the purpose of the visit to Caregiver Samantha Newell. At approximately 12:30 PM Administrator Nicolette Lake arrived. The LPA toured the exterior and interior of the facility. Ms. Lake explained to the LPA that the annual inspection had been completed in September of 2025. Instead, the annual inspection should be at their other facility, Oakley Assisted Living II 079201065. The LPA cancelled the inspection at this facility. No citations were issued during this inspection. Exit interview conducted and a copy of this report provided.
