What the inspector wrote

On 05/07/2026 at 10:25AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an unannounced annual 1-Year required inspection. LPA met with Brenda Williamson, Administrator, and explained the purpose of the visit. The administrator currently holds a certificate (#6055384740). The facility’s fire clearance was approved for six (6) non ambulatory clients. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) total bedrooms and three and a half (3 ½) bathrooms. One (1) bedroom used by staff. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 116.1 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Paper towel, and soap observed at all hand-washing stations. The supply of extra hygiene was available for residents. There are a minimum of 7-day non-perishables and 2-day perishables foods. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was purchased on 10/21/2025. Emergency Disaster Plan was last posted on 04/13/2026. Fire drill last conducted on 04/13/2026. First aid kit was observed to be complete. Continued on LIC809C… Continued from LIC809 LPA reviewed five (5) resident records and six (6) staff records, and they were current and complete. LPA requested the following documents to be submitted to CCLD by 05/14/2026. LIC 308 Designation of Administrative Responsibility LIC 309 Administrative Organization LIC 500 Personnel Report LIC 610E Emergency Disaster Plan LIC 9020 Register of Facility Clients/Residents Liability insurance Surety Bond Updated facility Sketch Continue on LIC809C.... Continued from LIC809C At 11:06AM, LPA observed facility has made alterations to bedroom #2 and bedroom #3 by installing a wall and door to separate the residents. LPA observed facility now has six (6) bedrooms five (5) for residents and one (1) for staff. Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights was provided to Brenda Williamson.
