What the inspector wrote

On 6/18/2025 at 9:00 AM, Licensing Program Analysts (LPA's) J. Clancy-Czuleger and A. Christy arrived unannounced to conduct a Required - 1 Year inspection. LPA met with caregiver, Gilberto Hernandez and explained the purpose of the visit. Administrator, Shan Gao arrived later. LPAs toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, garage, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be full and last serviced on 06/06/2025. Facility has sufficient towels, extra bed sheets and comforters. Equipment and supplies for residents' personal hygiene are available and on site. Dinner and silver wares were observed sufficient for residents' use. One week of nonperishable and 2-day of perishable food supplies were available. Hot water temperature was measured at 120.0 degrees F in the kitchen sink. LPAs observed grab bars and non-skid mat in the bathrooms. There were adequate lights in each room. First Aid kit is complete. Emergency Disaster plan was last reviewed 06/18/2025. Fire drills are conducted quarterly with the last date being 05/27/2025. LPAs reviewed 5 residents and 3 staff files, and 3 of 3 staff were fingerprint cleared and associated to the facility. No deficiencies cited during visit. Exit interview conducted and a copy of report was provided to the administrator.
