What the inspector wrote

On 04/12/2023 at 1:00 PM, Licensing Program Analyst (LPA) J Sampair arrived unannounced to complete the Post-Licensing Inspection that began 04/05/2023. LPA explained purpose of the visit to staff member Hilda Manuel who informed Licensee Mynette Boykin by phone. Licensee arrived at facility at 2:15 PM. 3 deficiencies cited during inspection. Inspection incomplete and will be continued at a future date. Exit interview conducted with Licensee. Copy of this report provided via email.
