What the inspector wrote

On 08/26/2025 at 10:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to complete the Required Annual Inspection began on 8/12/2025. Upon entry, the LPA stated the purpose of the visit to Caregivers Awan Lhemo and Noli Barrozo. The Administrator Shirley Largoza, Licensees Mylin Perdigue and Claro Villanueva arrived at approximately 11:00 AM. A detailed inspection of the physical plant had been completed on 8/12/2025 and the LPA observed no significant change since that date. The LPA reviewed the facility records and the records of 5 staff members. The Required Annual Inspection is complete. 1 Type-A and 1 Type-B citations were issued during the inspection (for details refer to LIC 809-D). Exit interview conducted and a copy of this report provided.
