Inspector’s narrative

What the inspector wrote

On 7/24/2024 at 10:15AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conducted a post licensing inspection. LPA met with Administrator, Imir Dzhudzho and explained the purpose of the visit. Upon entry, staff conducted COVID-19 screening for LPA. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and backyard. Facility has a 2-day supply of perishable and 7-day non-perishable food supplies. Grab bars and non-skid mats were observed in the residents’ bathrooms. Extra linens and towels were observed in the hallway closet. Smoke and carbon monoxide combination detectors were observed. There are no bodies of water observed. Medications were centrally stored and lock in a cabinet. Fire extinguisher was observed to be full. LPA reviewed 2 resident records and 2 staff records at around 12:00PM. LPA reviewed resident's medications. At 11:00AM, LPA measured hot water at 127.8 degrees F in the hallway bathroom. At 12:30PM, LPA observed R2's medical assessment stated R2 is bedridden. However, Administrator informed LPA that R2's doctor had changed R2's ambulatory status to non-ambulatory and waiting for updated documents. At 12:45PM, LPA observed S1 does not have current First Aid training on file. At 12:50PM, LPA observed S2 does not have health screening and TB test on file. At 1:00PM, LPA observed S2 does not have initial training on file. (Continue on LIC809C...) At 1:10PM, LPA observed S2 does not have medication training on file. At 1:30PM, LPA was informed by administrator that facility have not conducted a disaster drill. At 2:30PM, LPA observed R2's doctor's order for medications included Lactobacillus Combo and Multivitamins. However, facility ran out of the two medications and R2 have not been taking the two medications. The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and Health & Safety Code. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted. A copy of this report and appeal rights were provided.
