Pre-licensing visit·June 19, 2025

Licensing Program Analysts (LPA) J. Clancy-Czuleger and A, Christy, conducted an un-announced visit to the facility for the purpose of a Pre-Licensing evaluation. LPA's met with Caregiver Bryan Calpo and explained the purpose of the visit. This is pre-licensing visit is for a change of ownership. Licensee Suresh Reddy was called and he informed the LPA's that he is out of town and is not able to attend a meeting today. LPAs observed that the residents currently in care are in good health and two caregivers are working. LPAs informed Suresh Reddy that his attendance is needed for the Pre-Licensing evaluation and LPAs will make another attempt on another date. Exit interview conducted and a copy of this report provided.
