Pre-licensing visit·April 15, 2025

On 04/15/2025 at 11:15 AM, Licensing Program Analysts (LPAs) Y. Brown and J. Sampair, arrived to conduct an unannounced pre-licensing inspection of the facility. LPAs met with caregiver Ashley Ingleton and spoke with him regarding the purpose of visit. Administrators Telesha Clarke and Timea Yost arrived at the facility at 11:30 AM. The LPAs toured the interior and exterior of the facility. The LPAs inspected the kitchen, dining area, restrooms, community living spaces, bathrooms, resident rooms, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the living room was measured at 75.1 degrees Fahrenheit. Hot water temperature in the kitchen sink was measured at 119.3 degrees Fahrenheit. Fire extinguisher was fully charged and last serviced on 05/17/2024. The combination carbon monoxide and smoke detector was fully operational. Emergency Disaster Plan was posted and up to date. Pre-Licensing is incomplete with deficiency to be resolved by 04/22/2025. A follow up Pre-licensure LIC809 will be generated upon resolution of deficiency. LPAs completed COMP III training with Administrator. The exit interview was conducted and a copy of this report provided.
