Inspector’s narrative

What the inspector wrote

On 08/05/2025 at 9:30 AM, Licensing Program Analysts (LPAs) David Doidge and James Sampair arrived unannounced to conduct this post licensing inspection. LPAs met with Administrator Timea Yost and explained the purpose of the visit. LPAs toured the facility including but not limited to residents’ apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 70 degrees Fahrenheit. The hot water temperature was measured at 109 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2 days of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 06/02/2025. Emergency Disaster Plan was last posted on 11/01/2024. Emergency disaster and fire drill are conducted monthly; last conducted in May of 2025. First aid kit was observed to be complete. LPAs reviewed five (5) residents records For two (2) of five (5) residents, neither a Medical Assessment nor the Appraisal Needs and Services Plans were completed within 12 the past months. LPA reviewed four (4) staff records which were complete. Two (2) type-B deficiencies cited, For details information refer to LIC809-D. Exit interview conducted and a copy of this report provided.
