What the inspector wrote

On 04/27/2026, at 09:45 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Case Management - Deficiencies visit. Upon arrival, the LPA informed Manager Maria Hateley of the purpose of the visit. During the visit, the LPA cited the facility for the following one (1) Type B deficiency: B Citation – interior and exterior refrigerator/freezers above 40 degrees F and 0 degrees F respectively. Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC 809-D. Failure to submit Proof of Corrections (POCs) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted and a copy of this report provided.

On 04/27/2026, the inside refrigerator/freezer was 35.7 F and 6.8 F respectively, which serves as a potential risk to the health of residents in care.
