What the inspector wrote

On 04/24/2025 at 1:10PM, Licensing Program Analyst (LPA) Carol Fowler, conducted an unannounced annual 1-year required inspection. LPA met with Rochelle Sicat, Lead Caregiver, LPA explained the purpose of the visit. The facility’s fire clearance was approved for six (6) non-ambulatory and two (2) bedridden residents. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two and a half (2.5 ) bathrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Hot water temperature in the shared clients’ bathroom was measured at 108.5 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods. Smoke detectors/carbon monoxide were in operating condition during visit. The emergency disaster plan was last updated 4/24/2025. Fire extinguisher was purchased 1/31/2025. Fire drill last conducted 03/21/2025. First aid kit was observed to be complete. Continued on LIC809C. Continued from LIC809. LPA reviewed Three (3) residents files which were not complete. LPA reviewed Three (3) staff files which were incomplete. LPA observed the following deficiencies: · At 03:15PM, LPA observed a staff rest area being used for staff living quarters. · At 03:20PM, LPA observed incomplete resident files. · At 03:35PM, LPA observed staff files incomplete LPA requested the following documents to be submitted to CCLD by 5/02/2025. · LIC 308 Designation of Administrative Responsibility · LIC 309 Administrative Organization · LIC 500 Personnel Report · LIC 610E Emergency Disaster Plan (9 pages) · Liability Insurance The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in civil penalties. Exit interview conducted and a copy of this report and appeal rights provided
