On 01/27/2026 at 3:00PM Licensing Program Analyst (LPA) A Gomez conducted an unannounced Case Management visit regarding a self-reported incident. LPA met with Executive Director (ED), Kiel Stromgren, and explained the purpose of the visit. A self-reported incident report was received on 1/13/2026 from facility that indicated Resident 1(R1) was verbally threatened by Staff 1 (S1). During the visit, LPA collected the facilities internal investigation documents, S1's notice of resignation, and R1's admission documents. LPA also requested to interview S2 who was present during the interaction between R1 and S1 however they were not on shift. LPA requested contact information for all staff involved in incident be emailed by 2/1/2026. LPA will return to conduct additional interviews. No deficiencies cited during visit.
