What the inspector wrote

On 04/21/2026 at 2:30 PM, Licensing Program Analysts (LPAs) A. Gomez and Y Brown conducted a case management as a result of self reported incidents on 3/10/2026. LPAs met with Administrator, Brittany White and explained the purpose of the visit. Four (4) incidents were reported on 3/10/2026 for 3 separate occasions related to the same resident (R1). It was reported that on 2/22/2026 R1 had a fall. It was also reported that on 2/25/2026 R1 had a fall. Also on 3/4/2026 R1 had an additional fall and was found unresponsive and had passed away. The cause of death listed on the death report submitted by the facility was Unspecified Dementia with Psychotic Disturbance. It was observed that the fall on 2/22/2026, and 2/25/2026 where not reported to licensing within the required time frame of seven (7) days. The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties. Exit interview conducted. Appeal Rights and a copy of this report provided.

    87309 Storage Space and Access(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools...This requirement is not met as evidence by: Based on observation, the licensee did not comply with the section cited above in having unlocked chemicals in a residents room such as a multi-purpose cleaner, and disinfectant spray which poses an immediate health and safety risk to persons in care.

    87465 Incidental Medical and Dental Care(h)The following requirements shall apply to medications which are centrally stored:(2)Centrally stored medicines shall be kept in a safe and locked place that is not accessible...This requirement is not met as evidence by: Based on observation, the licensee did not comply with the section cited above in having unlocked medications in a residents room such as Clotrimazole and unlocked medications in the refrigerator such as Insulin Glargine-yfgn which poses an immediate health and safety risk to persons in care.

    (a) Each licensee shall furnish to the licensing agency such reports as the Department...to, the following:(1)A written report shall be submitted...within seven days of the occurrence of any of the events specified...(D) Any incident which threatens the welfare, safety or health of any resident,...or unexplained absence of any resident.This requirement was not met as evidece by: Based on record review, the licensee did not comply with the section cited above in not reporting the falls for R1 on 2/22/2026 and 2/25/2026 until 3/4/2026 which poses a potential personal rights risk to persons in care.
