Correction check·May 12, 2026

On 5/12/2026, at 12:00pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a proof of correction (POC) visit. LPA met with Jenna Asuncion, Caregiver, and explained the purpose of the visit. LPA spoke with Administrator Maria Matel, via telephone. LPA conducted an annual inspection on 4/28/2026 and cited for the following deficiencies that has not been corrected. 87463(a) - LPA observed service plans were not completed. LPA conducted an annual inspection on 4/28/2026 and cited for the following deficiencies that have been corrected. 87412(f) - LPA observed S1 file is available at the facility for review. 87217(g) - was cleared during initial inspection on 4/28/2026. 87412(a)(11) - S4 scheduled urgent physical and TB test on today's date at 1:04pm. Continued from LIC809C. 87307(a)(3)(c) - LPA observed on today's date that towels and linens were order on the Amazon app. 874555(b)(26) - LPA observed S1 placed order today during inspection to have food delivered to facility. *An immediate civil penalty of $700.00 will be assessed on today's date for regulation 87463(a) ($100.00 x 7)* Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties. Exit interview conducted.
