Pre-licensing visit·June 12, 2025

On 06/12/2025 at 10:00 AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an announced Pre-licensing Inspection. LPA met with Licensee, Clearnise Bullard and Administrator, Rochelle Thommes. LPA inspected the facility inside and out including but not limited to the bedrooms, bathrooms, common living areas, kitchen, garage, back yard. The facility has four (4) bedrooms and two (2) bathrooms. No bodies of water observed. There is sufficient lighting around the facility. Residents’ rooms are equipped with the proper furniture, bedding, and lighting. Bathrooms showers/tubs were equipped with non-skid mats. Passageways and hallways are free of obstruction. Locked cabinets available to store medications, toxins and sharps. Hot water temperature is measured at 118.6 degrees Fahrenheit in shared clients' bathroom. Fire extinguisher was last purchased on 01/27/2025. There is a minimum of 7-day non-perishables and 2-day perishables foods. First Aid kit was complete. Carbon monoxide and smoke detectors present. Facility inspection matches the sketch that was provided. This notification may be done by phone, by mail, or by fax . The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): 1.)LPA observed facility does not have a grab bar near toilet in one of the common restrooms. 2.)LPA observed facility does not have a grab bar in bathtub/shower common restrooms. 3.) LPA observed facility does not have an exit sign in the backyard identifying emergency exit. 4.)LPA observed facility does not have a covered area in backyard for residents in care. 5.) LPA reviewed dementia plan and observed the plan does not address behaviors such as ingestion of toxic chemicals and wandering behaviors. Licensee/Applicant will submit proof of corrections to CCLD on/before 04/29/2025 COMP III conducted with Licensee/ Administrator on 06/12/2025. Exit interview conducted and a copy of tis report provided.
