What the inspector wrote

On this date 09/25/2025 at 12:15 PM Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Post Licensing inspection. LPA met with caregiver, Sarah Duarte. Sarah phoned the Administrator, Alex Lago, to inform. Alex arrived approximately 30 mins later. Facility’s fire clearance was approved for six (6) residents. Hospice waiver approved for two (2). LPA inspected the facility with Alex including but not limited to the bedrooms, bathrooms, kitchen, dining area and the backyard. Facility had 2 (two) residents during the inspection. LPA observed a minimum of one week supply of non-perishable and 2-day perishable foods. Fire extinguishers were last inspected on 05/28/2025. First aid kit was complete with manual. Carbon monoxide and smoke detectors were observed operational during visit. Centrally stored medications, toxins and sharps were locked and inaccessible to residents. Grab bars and non-skid mats were observed in residents shared bathroom. A comfortable room temperature was maintained at 73 degrees F. Hot water temperature was measured at 120 degrees F. First aid kit was complete. LPA reviewed two (2) residents records. LPA reviewed 1 staff record and staff have current first aid training and associated to the facility. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
