What the inspector wrote

On 11/4/2025 at 10:50am, Licensing Program Analyst (LPA) L. Hall conducted an announced pre-licensing inspection. LPA met with Anthony Nzenwosu, Administrator/Licensee, explained the purpose of the visit. The facility has an approved fire safety clearance for six (6) ambulatory residents. LPA inspected the facility inside and out including but not limited to the bedrooms, bathrooms, common living areas, kitchen, garage, back yard. The facility has a four (4) bedrooms and two (2) bathrooms. One (1) bedroom will be used by staff. No bodies of water observed. There is sufficient lighting around the facility. Passageways and hallways are free of obstruction. Locked cabinets available to store medications, toxins and sharps. Hot water temperature is measured at 107.1 degrees F. in shared clients' bathroom. There is a minimum of 7-day non-perishables and 2-day perishables foods. Fire extinguisher was purchased on June 2025. Carbon monoxide and smoke detectors present and operable. Hot water in shared residents' bathroom measured at 98.6 degrees F. The following will need to be corrected by 11/10/2025: Purchase linen, which shall include towels, sheets, blankets, bedspreads. Purchase chest of drawers for resident rooms. Continued on LIC809C. Continued from LIC809. Updated facility sketch to show bathroom in staff room. Purchase non-skid mats for bathroom. Purchase first aid kit and emergency lighting. *LPA will conduct the COMP III upon return to complete pre licensing inspection* Issues were noted during inspection. LPA observed that facility is not ready to be licensed. This report will be submitted to the Central Applications Unit (CAU) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAU. Additional requirements may still be required. Exit interview conducted and a copy of this report provided.
