Pre-licensing visit·November 13, 2025

On 11/13/2025 at 10:30am, Licensing Program Analyst (LPA) Carol Fowler conducted an announced pre-licensing follow-up inspection. LPA met with Anthony Nzenwosu, Administrator/Licensee, explained the purpose of the visit. The facility has an approved fire safety clearance for six (6) ambulatory residents. LPA conducted a follow-up inspection for corrections inspected LPA toured bedrooms and bathroom and observed the following linen, towels, sheets, blankets, and bedspreads. Each resident room contained chest of drawers. LPA observed the updated facility sketch to show bathroom in staff room, non-skid mats in the residents bathroom, first aid kit is complete and emergency lighting. No Issues were noted during inspection. LPA observed that facility is ready to be licensed. This report will be submitted to the Central Applications Unit (CAU) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAU. Additional requirements may still be required. Exit interview conducted and a copy of this report provided.
