What the inspector wrote

Licensing Program Analyst (LPA) A. Gomez and Licensing Program Managemer (LPM) arrived announced to conduct a Post Licensing inspection on this date starting at 9:30 am. LPA and LPM met with Administrator Mahdokht Dorie Ofogh. Facility’s fire clearance was approved for six non-ambulatory of which 1 may be bedridden in room 2 only. LPA inspected the facility with Administrator Mahdokht Dorie Ofogh including but not limited to the bedrooms, bathrooms, kitchen, dining area and the backyard. Facility had 0 residents during the inspection. LPA observed a minimum of one week supply of non-perishable and 2-day perishable foods. Fire extinguishers were last purchased in July 2025. First aid kit was complete with manual. Carbon monoxide and smoke detectors were observed operational during visit. Auditory signals were installed in each exit doors and observed functioning during visit. Centrally stored medications, toxins and sharps were locked and inaccessible to residents. Grab bars and non-skid mats were observed in residents shared bathrooms. Hot water temperature was measured at 119.5 degrees F. Administrator states that first residents are set to move in June 1. 2026 Exit interview conducted with Administrator Mahdokht Dorie Ofogh and copy of this report provided.
