Pre-licensing visit·October 15, 2025

On 10/15/2025 at 1:00PM, Licensing Program Analyst (LPA) Carol Fowler conducted a Pre-licensing Inspection. LPA met with Caregiver, Gleceria Matabang and explained the purpose of the visit. Staff contacted Administrator and Administrator is on the way to the facility. Administrator arrived at 2:00PM. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, dining area, kitchen, garage, and outdoor area. LPA observed lighting in all rooms. Hot water was measured at 111.6 degrees F in the hallway bathroom. Facility bathrooms have grab bars and non-skid mat/materials for the showers. LPA observed facility have one week of non-perishable and two day of perishable food supplies available. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be expired and last serviced on 09/07/2024. First aid kit was complete. No bodies of water observed. Last disaster drill was conducted on 08/02/2025. LPA observed the following ·Wood and screens stored behind the shed. ·Shower floor in bathroom #3 needs repair/paint. .Bathroom #1 slow drain, cabinet doors needs repair, lose knobs. CONTINUE ON LIC809C CONTINUE FROM LIC809 Issues were noted during inspection. LPA observed that facility is not ready to be licensed. This report will be submitted to the Central Applications Bureau (CAB) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAB. Additional requirements may still be required. Exit interview conducted and a copy of this report provided.
