Inspector’s narrative

What the inspector wrote

On 8/14/2025, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a post licensing inspection. LPA met with Caregiver Reymund Tulabot. Administrator, Liwayway Mendoza, arrived at 2:10pm, and LPA explained the purpose of the visit. The facility’s fire clearance was approved for six (6) non-ambulatory residents. LPA inspected the facility with Caregiver including but not limited to the bedrooms, bathrooms, kitchen, common area, storage areas, and the backyard. LPA observed all bedrooms were equipped with required furniture for the comfort and safety of the residents. There are grab bars for each residents’ shower and toilet. Hallways and passages are free of obstruction. Hygiene supplies and toiletries were available. Facility temperature is maintained at 71 degrees F. Hot water temperature measures 122.5 degrees F. in residents' shared bathroom. Fire extinguishers were last inspected on 6/10/2025. First aid kit was complete with manual. Carbon monoxide and smoke detectors were observed operational during visit. Facility inspection matches the sketch that was provided. LPA reviewed four (4) staff and four (4) Resident records all were complete. Continued on LIC809C. Continued from LIC809. LPA observed the following deficiencies: At 1:52pm, LPA observed medication in unlocked kitchen cabinet drawer, and knives and scissors in a different unlocked cabinet drawer. At 2:00pm, LPA observed planks of wood, 6 5-gallons of paint, bed rails, 2 glass shower doors, and a wheel chair in back yard. At 2:05pm, LPA observed pills sitting on a napkin on R1's tray beside the bed. Deficiencies were cited from Title 22 California Code of Regulations (see 809D). Failure to submit proof of corrections by plan of correction due date, and any repeat violations within 12-month period may result in civil penalties. Exit interview conducted. A copy of appeal rights and this report provided.
