Pre-licensing visit·November 19, 2025

On 11/19/2025 at 10:11 AM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived announced to conducted a Pre-licensing Inspection. LPA met with Licensee, Gerardo Guerrero and Administrator, Imelda Guerrero. LPA toured the facility with Licensee including but not limited to the residents' bedrooms, bathrooms, kitchen, common areas, garage, and outdoor area. LPA observed facility has sufficient lighting throughout the facility. Indoor and outdoor passageways were observed free of obstruction. There were no bodies of water observed. LPA observed 2 days’ supply of perishable foods and 7-day supply of non-perishable foods. The facility consists of five (5) bedrooms one (1) bedroom occupied by staff, and two (2) bathrooms. LPA observed facility providing care to family member in room #1. LPA observed non-skid mats and grab bars in bathrooms. There is activity materials observed in the living room. Facility has flashlights available for emergency use. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was purchased on 06/03/2025. Proper hand-washing signs, Emergency/Disaster plans/contact information, personal rights were observed posted in common areas. Continue on LIC809C..... Continued from LIC809 The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): 1. LPA observed facility does not have a dementia care plan in place. 2 LPA observed facility does not have mattress pads and blankets for all six (6) beds. 3. LPA observed facility does not have telephone service on the premises. 4. LPA observed facility does not have a properly locked cabinet with disinfectants and detergent. 5. LPA observed facility does not have a properly locked shed with tools and paint located in the backyard. 6. LPA observed facility does not have linen in sufficient numbers to support weekly changing. 7. LPA observed facility does not have a window screen on bedroom#2 window. 8. LPA observed facility does not have signal system. 9. LPA observed facility does not have any exit signs throughout the facility . Licensee/Applicant will submit proof of corrections to CCLD on/before 11/26/2025. COMP III will be conducted during next visit Exit interview conducted and a copy of this report provided.
