What the inspector wrote

On 11/26/2025 at 10:00AM, Licensing Program Analyst (LPA) T. Syess-Gibson conducted an announced pre-licensing inspection. This is a continuation from the previous visit conducted on 11/19/2025. LPA met with Licensee, Gerardo Guerrero and Administrator, Imelda Guerrero and explained the purpose of the visit. On 11/19/2025 LPA toured the facility with Licensee including but not limited to the residents' bedrooms,bathrooms, kitchen, common areas, garage, and outdoor area. LPA observed facility has sufficient lighting throughout the facility. Indoor and outdoor passageways were observed free of obstruction. There were no bodies of water observed. LPA observed 2 days’ supply of perishable foods and 7-day supply of non-perishable foods. The facility consists of five (5) bedrooms one (1) bedroom occupied by staff, and two (2) bathrooms. LPA observed facility providing care to family member in room #1. LPA observed non-skid mats and grab bars in bathrooms. There is activity materials observed in the living room. Facility has flashlights available for emergency use. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was purchased on 06/03/2025. Proper hand-washing signs, Emergency/Disaster plans/contact information, personal rights were observed posted in common areas. Continue on LIC809C... Continued from LIC809 On 11/26/2025 LPA toured the facility and observed Licensee/Administrator made corrections from previous visit on 11/19/2025. LPA Conducted COMP III on 11/26/2025. The licensee/administrator was reminded of the statute that requires CCL to be notified within 5 business days of admitting their first resident. This notification may be done by phone, by mail, by email, or by fax. No Issues were noted during inspection. LPA observed that facility is ready to be licensed. This report will be submitted to the Central Applications Unit (CAU) and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAU. Additional requirements may still be required. Exit interview conducted and a copy of this report provided.
