What the inspector wrote

On 12/10/2025 at 9:35 AM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived announced to conduct a Pre-licensing Inspection. LPA met with Licensee, Satinder Kaur Antal and Administrator, Harpreet Singh. Facility’s fire clearance was approved for four (4) non ambulatory and two (2) bedridden residents. LPA toured the facility with Licensee including but not limited to the residents' bedrooms, bathrooms, kitchen, common areas, garage, and outdoor area. LPA observed facility has sufficient lighting throughout the facility. Indoor and outdoor passageways were observed free of obstruction. There were no bodies of water observed. LPA observed some perishables and nonperishable foods; licensee will purchase more food once licensed. The facility consists of seven (7) bedrooms one (1) bedroom occupied by staff, and three (3) bathrooms. LPA observed non-skid mats and grab bars in bathrooms. Locked cabinets available to store medications and toxins. Hot water temperature is measured at 145.1 degrees Fahrenheit. There is activity materials observed in the living room. Facility has flashlights available for emergency use. First-aid kit was observed to be complete. Facility inspection matches the sketch that was provided. Continue on LIC809C..... Continued from LIC809 Smoke detectors and carbon monoxide were operational. Fire extinguisher was purchased on 10/13/2025. Proper hand-washing signs, Emergency/Disaster plans/contact information, personal rights were observed posted in common areas. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): 1.) LPA observed facility does not have a dementia care plan addressing wandering, elopement and ingestion of toxins behaviors. 2.) LPA observed facility does not chest of drawers in residents’ bedrooms. 3.) LPA observed facility does not have curtains or blinds to window in one bedroom. 4.) LPA observed facility does not have window screens in master bedroom. 5.) LPA observed water temperature measured at 145.1 -degrees F. 6.) LPA observed facility does not have a adequate shady area for residents. 7.) LPA observed facility does not have telephone service. Licensee/Applicant will submit proof of corrections to CCLD on/before 12/17/2025. COMP III will be conducted during next visit. Exit interview conducted and a copy of this report was provided.
