What the inspector wrote

On 11/17/2025 at 9:30AM, Licensing Program Analysts (LPAs) Grace Luk and Andrew Christy conducted a Pre-licensing Inspection. LPAs met with Licensee/Applicant, Dorie Ofogh, and explained the purpose of the visit. There are currently no residents at the facility. LPAs toured facility including but not limited to resident's bedrooms, bathrooms, living room, dining area, kitchen, garage, and outdoor area. LPAs observed lighting in all rooms. A comfortable indoor temperature is maintained at 70.0 degrees Fahrenheit. Hot water in the residents' bathroom was measured at 120.0 degrees Fahrenheit. Facility bathrooms have grab bars and non-skid mat/materials for the showers. LPAs observed facility have some non-perishable and perishable food supplies available. Carbon monoxide and smoke detectors are in operating condition. Fire extinguisher was purchased 07/23/2025. First aid kit was complete. The landline telephone in the living room was operational and able to make calls. Emergency Disaster Plan is on site and was reviewed 10/25/2025. There is also an Infection Control Plan and Plan of Operations on site. Liability insurance will be purchased once licensed. Continued on LIC809C..... Continued from LIC809..... The following will need to be completed/fixed before recommending licensure to the Central Applications Bureau (CAB): The original facility sketch states there is a detached garage on the side of the facility. However, during inspection, the detached garage was under construction to become an Accessory Dwelling Unit (ADU) and not fully completed. LPAs and Licensee will follow up with the Fire Department as necessary. The side path next to the uncompleted detached garage is lined with loose rock. Per Licensee, a concrete path will be created. Additionally, the side of the detached garage was the only outdoor exit from the backyard area. LPAs and Licensee will follow up with the Fire Department as necessary. There was a hole above the door that leads to bedrooms 1, 2, and 3 with some exposed wires. Rooms 1 and 4 also need screen doors. There is no shower curtain in the hallway bathroom used by rooms 2, 3, and 4. In addition, there is no shower curtain or wall for the private bathroom in room 1. The rooms are not fully furnished. There is no chest of drawers in rooms 1 and 2. Room 2 is missing chairs. Room 1 needs another lamp. The CCLD poster is undersized and needs to be 20" by 26" at minimum. There should be a bulletin board with important facility documents and information in an accessible area. The First Aid kit is missing a thermometer. Licensee will submit proof of corrections to LPAs on or before 12/01/2025. Exit interview conducted, and a copy of this report was provided to the Licensee.
