What the inspector wrote

Prior to entering the facility, LPA Smith spoke with staff to pre-screen that the facility is COVID free. Analyst also self-screened for having no known symptoms or exposure. Analyst followed facility's screening protocols, wore a mask and maintained distance during the visit. LPA Smith conducted an unannounced complaint visit and met with Angela Martinez. Based on photographic / medical documentation and witness testimony, this allegation is SUBSTANTIATED. LPA observed evidence which showed uncut toenails and unbrushed teeth on R1. These conditions could only occur due to a lack of hygienic care on a routine basis. Additional documentation includes medical discharge paperwork where the medical directive was "to keep mouth clean". As a result of this investigation, LPA finds the allegation that resident is not receiving the proper care needed to be SUBSTANTIATED. A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. The following deficiencies were cited on 9099-D, per Title 22 Regulations, Division 6.

This requirement is not met as evidenced by: Based on photographic evidence, witness interviews and medical documentation, licensee did not observe that R1s teeth and toenails had been neglected over a period of time and needed attention. This is in violation of this section. This poses a potential health and safety risk to residents in care.
