What the inspector wrote

Licensing Program Analyst (LPAs) Lady Cabrera conducted a subsequent Case Management visit to discuss information obtained from the initial visit conducted on 09/17/2021. LPA met with Administrator Shaun Rushforth and stated the purpose of the visit. LPA was provided with records regarding the facility’s internal investigation regarding Staff (S1) witnessing potential abuse by S2 onto Resident (R1). During the internal investigation, it was determined based on staff and resident interviews no abuse had taken place. No follow-up required. Exit interview was conducted. Administrator was provided with a copy of this report.
