Record dose details and resident medication response

Based on records reviewed and interview conducted with Licensee, all residents medications were administered daily. All residents MAR is not recorded when medications administered from 08/04/25 to 08/07/25 in the evening, which poses/ posed a potential health and safety risk for the person in care.

Provide resident hot water for personal care

Based on observation, the licensee did not comply with the section cited above when hot water measured at 102.5 degrees F which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain facilities for fire and panic safety

Based on observation, the licensee did not comply with the section cited above when the fire extinguisher was last serviced on 08/25/2022 which poses an immediate health, safety or personal rights risk to persons in care.

Provide resident hot water for personal care

Based on observation, the licensee did not comply with the section cited above in when hot water measured at 99.5 degrees F in bathroom 1, 99.1 degrees F in bathroom 2, and 97.5 degrees F in bathroom 3 which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain records of centrally stored medication dosages

Based on record review, the licensee did not comply with the section cited above when the medication records were observed to be incomplete, which poses/posed a potential health, safety or personal rights risk to persons in care.

Label and preserve medicine compliance standards

Based on observation and record review, the licensee did not comply with the section cited above when resident prescription labels were altered which poses/posed a potential health, safety or personal rights risk to persons in care.

Arrange appropriate medical and dental care

Based on record review, the licensee did not comply with the section cited above when 5 out of 5 residents did not have a needs & services/ care plan, which poses/posed a potential health, safety or personal rights risk to persons in care.

Keep prescriptions in original containers

Based on observation and record review, the licensee did not comply with the section cited above when medicaitons were observed to be transferred between containers which poses/posed a potential health, safety or personal rights risk to persons in care.

Authorized physician-prescribed self-administered meds

Based on record review, the licensee did not comply with the section cited above when R2 has been receiving an over the counter medication that has not been prescribed by a physician, which poses an immediate health, safety or personal rights risk to persons in care.

Maintain facility in clean, safe, sanitary condition

Based on observation, the licensee did not comply with the section cited above. LPA observed black mold in east hall bathroom bathtub caulking, in caulking of tiles around bathtub spout, and soap scum on tile wall surround; black mold in grout lines of tile wall surround in west hall bathroom; and east hall bathroom shower head observed with brown residue, which poses a potential health or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. Hot water in east hall bathroom measured at 100.9 degrees F, which poses a potential health, safety or personal rights risk to persons in care.

Maintain facilities for fire and panic safety

Based on observation, the licensee did not comply with the section cited above. Fire extinguisher last purchased 10/8/2020, which poses an immediate safety or personal rights risk to persons in care.
