Give PRN medication by physician order

Based on observation and records reviewed, Medication was checked. LPA and Licensee observed R1 and R2’s medications were not administered as directed by physician, which poses/posed an immediate health and safety risk for the person in care.

Based on interview conducted, observation, records reviewed, R2 is not receiving hospice care, was observed lying bed using a hospital bed with full rail with doctor’s order, which poses/posed a potential health and safety and personal risk to the resident in care.

Maintain physician order documentation in resident record

Based on interview conducted, observation, records reviewed, R1 has an electric hospital bed with no doctor’s order and R3 has a half rail bed with no doctor’s order, which poses/posed a potential health and safety and personal risk to the resident in care.

Based on records reviewed, all the residents was not observed with a needs and services plan on file, which poses/posed a potential health and safety and personal risk to the resident in care.

Record centrally stored prescriptions and refill data

Based on observation and records reviewed, R1’s medication Folic Acid 1 mg and Clotrimazole. R2’s medication Folic Acid 1 mg, Glipizide 10mg, and Levothyroxine 0.025mg, and R3’s medication Azelastine 0.1% were not record in Centrally Stored Medication (Lic 622) record, poses/posed a potential health and safety and personal rights risk to theresident in care.

Record dose details and resident medication response

Based on records reviewed and interview conducted with Licensee, all residents medications were administered daily. MAR is not recorded when medications administered, which poses/ posed a potential health and safety risk for the person in care.

Ensure hazardous items are locked and not unattended

Based on observation, LPA and L1 observed a knife unlock in kitchen drawer unlock accessible to the residents, which poses/posed an immediate health and safety risk for the person in care.

Based on observation the licensee did not comply with the section cited above when full bed rails were observed to be equipped on a resident's bed without a physician's order, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above when the facility was unable to provide documetation that annual training has been conducted, which poses/posed a potential health, safety or personal rights risk to persons in care.

Authorized physician-prescribed self-administered meds

Based on record review, the licensee did not comply with the section cited above when staff administered over-the-counter medication to residents in care which had not been authorized by the person's physician, which poses/posed a potential health, safety or personal rights risk to persons in care.

Store centrally held medications in locked secure place

Based on observation, the facility did not ensure medicines were kept in safe and locked place when staff placed medication meant for R2 in a dispensing cup near R1, which poses an immediate health, safety or personal rights risk to persons in care.

Passageways and stairways kept clear

Based on observation, the licensee did not comply with the section cited above, when LPA observed various equipment to be blocking an exit in the backyard and a chair furniture blocking the exit in the living room, which poses an immediate health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on observation, the licensee did not comply with the section cited above when knives were observed on a container in the kitchen accessible to residents in care and a bottle of disinfectant was observed under sink the bathroom which posed a potential health, safety or personal rights risk to persons in care.

Submit and maintain current mailing address

87355 Criminal Record Clearance (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1569.17(b) shall prior to working... (1) Obtain a California clearance or a criminal record exemption as required by the Department orThis requirement is not met as evidenced by: S2 did not have fingerprint criminal clearance and was present and working. S2 advises this is S2's first day of work and had not yet started the fingerprint clearance process. This poses an immediate health, safety, or personal rights risk to residents in care.

S1 was present and working. S1 has been working in the facility since 11/3/22 and has worked three days which poses an immediate health, safety, or personal rights risk to residents in care.

Maintain facility in clean, safe, sanitary condition

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.This requirement is not met as evidenced by: Excess furniture, medical equipment, and miscellaneous items observed stored in area outside on the north side of the facility and also in the garage, which poses a potential health, safety, or personal rights risk to residents in care.

87309 Storage Space (a) Disinfectants, cleaning solutions, poisons, firearms and other items which could pose a danger if readily available to clients shall be stored where inaccessible to clients.This requirement is not met as evidenced by: Blender blade observed still in blender on counter and one steak knife observed in kitchen sink and accessible, which poses an immediate safety or personal rights risk to residents in care.

Passageways and stairways kept clear

Based on observations, the licensee did not comply with the section cited above. Baby gate is installed between dining room and kitchen and passageway between foyer and kitchen. Treadmill observed stored in passageway between foyer and kitchen, all which poses a potential safety or personal rights risk to persons in care.

Maintain facility in clean, safe, sanitary condition

Based on observations and interviews, the licensee did not comply with the section cited above. LPA observed hallway and master bathroom tile shower walls and floors with mold and soap scum, sliding screen doors for both master bedroom and living room observed in disrepair and not sliding with ease, molded lemons and tomatoes observed in bottom drawer in refrigerator, fire exit gate observed with stiff metal wire as pull chain, excess electric cords in dining room strewn about in fireplace corner, and excess furniture, medical equipment, mattresses, and miscellaneous items observed in back patio and in garage, which poses a potential health, safety or personal rights risk to persons in care.

Report specified resident events within seven days

Based on interviews, the licensee did not comply with the section cited above. On 6/28/22, all four residents tested positive for COVID-19 and the Administrator did not submit an incident report to CCL, which posed a potential health, safety or personal rights risk to persons in care.

Maintain facilities for fire and panic safety

Based on observation, the licensee did not comply with the section cited above. All fire extinguishers in the facility, total of 2, were last serviced on 9/12/2020, which poses an immediate health or safety risk to persons in care.

Fire approval and staff access to unlock systems

Based on observations and interview, the licensee did not comply with the section cited above. LPA observed six bottles of prescribed medications accessible on top of kitchen counter, which poses an immediate health, safety, or personal rishts risk to persons in care.

Notify agency before locking doors or gates

Based on observations, the licensee did not comply with the section cited above in. LPA observed a rusted shovel outside by fire exit gate; a shovel inside garage by side exit door; a steak knife and a pair of scissors on kitchen counter; hall closet where toilet cleaner, a can of Comet cleaner, a bottle of cleaner, and a can of disinfecting spray was unlocked and accessible; a bottle of Pine Sol cleaner, spray bottle of cleaner, bottle of toilet cleaner, and can of tung oil finish were observed accessible and sitting on top of washing machine in laundry area that was accessible; and an open can of Comet cleaner, spray bottle of cleaner, large bottle of weed killer, bottle of stain odor remover, and a bottle of charcoal lighter fluid was observed accessible in garage, which poses an immediate health, safety or personal rights risk to persons in care.

Separate storage for cleaning chemicals

Based on observations, the licensee did not comply with the section cited above. LPA observed an open can of Comet cleaner, two large bottles of Pine Sol cleaner, a bottle of charcoal lighter fluid, and a spray bottle of cleaner stored in open area below a small table in garage where several bags of spaghetti, a medium sized plastic container filled with white rice, and several canned food items were kept, which poses an immediate health, safety or personal rights risk to persons in care.
