What the inspector wrote

Allegation: Staff is isolating and neglecting resident. R1 stated staff makes them eat meal isolated in a bedroom. Based of observation, interviews and record reviews facility serves meals in main dining room where all residents share meal time. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. No deficiencies were cited. Exit interview conducted, report signed and copy of this report provided to the Administrator for facility records.

This was not observed as evidenced by: The facility failed to provide the Licensing department with R1 refusal of medication intake on multiple occations: 2/9/24, 2/10/24, 2/11/24, 2/12/24, 2/17/24, 2/18/24, and 2/19/24 wich poses potential safety risc to residents in care.
