On 06/23/2021, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator (ADM) Susan Sundari. Visitor log-in/temperature check was observed upon entry. Facility has one entrance/exit point. Facility appeared cleaned with no obstruction or fire clearance issues. Hand sanitizer was available to residents and visitors. Social distancing is maintained in the common and dining areas. Hand washing posters were observed by the bathroom sink. Bedrooms were checked. Beds were approximately 6 feet apart in the shared bedroom. Facility was recently painted, signs promoting social distancing, cough/sneeze etiquette will be replaced throughout the facility. LPA checked residents’ locked medications. Food supply was checked, LPA observed a 7-day supply of non-perishables and a 2-day supply of perishables. Cleaning and PPE supplies were checked. Facility does not have an adequate supplies of required PPE. Staff records were reviewed for good health and infection control training. Facility staff was observed with mask on. Residents wear masks when away from the facility. Resident’s files have updated emergency contact information. Administrator certification is current. No deficiencies issued during this inspection. Exit interview was conducted. As a COVID-19 precautionary measure, a copy of this report will provided to ADM via email and an electronic read receipt confirms receiving this document.
