Routine inspection·December 29, 2025

On 12/29/25, Licensing Program Analyst (LPA) Daiquiri Boyd arrived at the facility unannounced to conduct a required annual visit. LPA was granted entry by caregiver Irish Bobo and LPA explained the purpose of the visit. Administrator Carlo Santos arrived at the facility shortly after. The residence was set at 73 F temperature and free of passageway obstructions inside and outside. LPA observed four bedrooms in the residence. Residents' rooms were toured and inspected. Rooms were found to be clean, and furnishing were in good condition. Hot water temperature was measured at 117 degrees F. Kitchen toured, supply of food observed, and food stored properly for perishable and nonperishable. Medications are locked in a cabinet in the shared living area. Knives are locked in the kitchen area. Cleaning supplies are locked under the kitchen sink and extra supplies are locked in the garage. Smoke detectors and carbon monoxide were checked and operating. Fire extinguishers were charged and was serviced on 05/02/2025. Emergency disaster drills are conducted quarterly, last drill completed on 10/16/2025. First Aid Kit was observed to be complete and in order. Infection Control Plan was observed and last reviewed 8/15/2025. There was outdoor seating for the residents. Outdoor area was clean and free of obstruction. An exit interview was conducted, and a copy of this report was provided to Administrator whose signature confirms receipt. LPA requested the following updated forms faxed to CCLD by 0106/2026: Personnel Report (LIC 500), Proof of current Liability Coverage.
