On 10/6/2023 Licensing Program Analyst (LPA) M. Garza arrived at facility to deliver complaint findings. LPA met with Direct Care Staff, Alma Alcantara and Licensee, Delia Galvez explained reason for visit and was permitted entry into the facility. A health and safety check was completed for residents in care. Residents observed in rooms and in common areas. Tour of facility was completed inside and out. During the investigation LPA completed interviews with staff and resident(s) and reviewed documentation (physicians reports and pest control receipts). Records reviewed indicated the facility has pest control services. Althought the allegation may or may not have occurred it does not meet the preponderance of evidence standard per Title 22. The allegation is UNSUBSTANTIATED. No deficiencies cited during this visit. Exit interview completed with Licensee, Delia. A copy of this report given.
