On 09/12/2022, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct a Case Management inspection. LPA introduced self, stated the purpose of the visit and met with Standards Compliance Coordinator, Cheryl Cardoza and Assistant Hospital Administrator, Thai Nguyen. The purpose of today's visit is to follow up on an incident that was reported to the Fresno CCL office. No deficiencies issued during today's inspection. Exit interview conducted. A copy of this report was discussed and provided to Standards Compliance Coordinator, Cheryl Cardoza whose signature on this form confirms receipt of this document.
