On 1/8/2026 Licensing Program Analyst (LPA) arrived to the facility unannounced to conduct a case management visit. LPA met with Administrator Scott Richards. The Dept received incident reports stating multiple residents tested positive for COVID. The Administrator stated the facility followed their infection control plan to help prevent further spread of the virus. The local Health Dept was notified and informed of the situation. Administrator stated no residents had to be sent to the hospital, and currently there is no active COVID cases at the facility. Exit interview was conducted and a copy of this report LIC809 was emailed to Administrator Scott Richards
