What the inspector wrote

Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct the Annual inspection. LPA met with and explained the purpose of the visit with Administrator (AD) Shelly Yarborogh and members of the Multi-disciplinary Team. During this visit, LPA toured the facility inside & out. Resident rooms were found to be clean and contained required furniture, bedding, personal items and are well lit. Bathrooms were clean and in good repair with safety requirements in place and hygiene items properly stored. Extra linens and towels were observed and available for use. Resident bathroom faucets deliver hot water as required. The kitchen was clean, in good repair with necessary items and appliances. LPA observed perishable and non-perishable food supply and paper products. Knives and cleaning/disinfecting supplies are locked and stored away from food. Centrally Stored medications are properly stored and locked. First aid kits contain required items. The facility has multiple visitation areas available inside and out. Doors and passageways are unobstructed throughout the home, including outdoors. The pool was gated as required and locked. The Fire Extinguishers were serviced 10/2025 by Valley Fire Co. LPA conducted resident and staff file reviews, P&I and a medication audit. Emergency Disaster and Infection Control Plans were also reviewed. There were no citations issued during this inspection. An exit interview was conducted, and a copy of this report was provided. LPA requested the following updated forms faxed to CCLD by11/17/25: Designation of Facility Responsibility (Lic308), Administrative Organization (Lic309), Personnel Report (LIC 500), Client Roster (LIC 9020) and Proof of current Liability Coverage
