What the inspector wrote

On 09/15/25, Licensing Program Analyst (LPA) L. Salazar arrived to the facility unannounced to conduct the required Annual Inspection Visit. LPA was greeted by staff stated the purpose of the visit and was allowed entry into the facility. Administrator on record is Laura Edwards. LPA toured the facility inside and out and observed the house to be a 4 bedroom, 3 bathroom house. LPA observed 4 residents in care at the time of visit. 3 out 4 residents were observed in the living room, sitting in individual recliners, watching game shows on TV. 1 out of 4 residents are receiving Hospice care services. 1 out of 4 residents are receiving home health services. 2 out of 4 residents are receiving Regional Center services. The facility temperature read at 76 degrees F. Resident bedrooms were observed to have the required lighting/furnishings and are free from odor and passageway obstruction or fire hazards. Mattresses and linen were observed in good condition. A supply of clean linens/towels were observed in a closet off the hall. Bathrooms were observed to have operational lights, running water, required grab bars and non- slip floors. Hot water temperature tested at 112 degrees F. Cleaning supplies and knives were observed to be locked cabinet located in the laundry room. LPA toured the kitchen observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. Menus for the facility were observed. LPA observed fire extinguisher with a service date of 09/08/25. Smoke alarm detectors and carbon monoxide detectors were observed to be operational. Last Disaster Drill was a fire drill on 08/20/25. First aid kits were observed to contain all required items. Medications were observed to be in a locked cabinet located the laundry room. (Continued on 809-C) (Continued from 809) Residents’ files were reviewed and observed to have update emergency contacts, Admission agreement, and current physician report. A sample of staff files were also reviewed. Staff files were observed to have current First Aid/CPR. Staff are fingerprinted clear and associated to the facility. Quarterly Emergency Disaster Drill logs were observed for staff. LPA observed on the LIC 610E (Emergency Disaster Plan) with emergency numbers and evacuations locations was posted in the hallway. An exit interview was conducted with Administrator. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit. LPA requested the following documents be submitted to the Fresno CCL office by 09/26/2025: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents for LIC9020, and proof of Liability insurance.
