Incident investigation·February 6, 2026

On February 6, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management for a visit previously reported incident. LPA was allowed entrance into the facility by Caregiver Maria Avila Ochoa. LPA spoke with House Manager Katelyn Hernandez via phone. LPA observed the facility to be clean, odor free, and clutter free. LPA explained the reason for the visit was due to an incident that was reported to the Dept. about R1 waking up and having bruising on their face. LPA spoke with House Manager Katelyn via phone. Katelyn stated R1 has been more agitated towards staff. Katelyn explained R1 might have hit their head/face on the bed rails which may have caused the bruising. Katlyn stated the family was inform and the bed rails have been cushioned with blankets. LPA did not observe any bruising on R1's face during the visit. Caregiver Maria stated R1's agitation has increased. LPA did not observe any deficients at this time and no citations were issued. Exit interview was conducted and a copy of this report LIC809 was provided to caregiver Maria.
