Record review of R1’s file and interviews reveal that R1 was experiencing increased confusion. The facility had met with and planned with R1’s Responsible Party to move R1 from Assisted Living to Memory Care upon return from a hospitalization due to recent change of condition. Facility Narrative Charting show that the facility was in communication with the family regarding falls and interventions put in place. Based on record review and interview, it is not determined that the resident sustained a fracture due to lack of care and supervision by the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. The allegation is UNSUBSTANTIATED. No deficiencies cited. A copy of this report was provided, and an exit interview was conducted with AD.
