What the inspector wrote

Licensing Program Analyst (LPA) Daiquiri Boyd arrived at the facility unannounced to conduct a required annual visit. LPA was granted entry by staff and explained the purpose of the visit. Licensee/Administrator Marilen Gonzales was called and spoke with LPA by phone and stated she couldn't come to the home due to a meeting. LPA completed the inspection with staff and Administrative Assistant, Ann Lorio. The residence was set at 73 degrees F temperature and free of passageway obstructions inside and outside. LPA observed five bedrooms in the residence. Residents' rooms were toured and inspected. Rooms were found to be clean, and furnishing was in good condition. Hot water temperature was measured at 110.8 degrees F. Smoke and Carbon monoxide detectors were observed and checked. Kitchen toured, supply of food observed, and food stored properly for perishable and nonperishable. Medication and knives are locked in a kitchen cabinet. Cleaning supplies were locked in the hallway closet. Smoke and carbon monoxide are dual detectors, they were checked and operating. Fire extinguishers was service on 2/5/2026. Last drill completed on 02/15/2026. LPA did not observe emergency food or water.There was outdoor seating for the residents. Outdoor area was clean and free of obstruction. Facility has a pool which is gated, locked and inaccessible to residents in care. Resident and staff records were reviewed. Medications were reviewed. Facility does not have any residents on Hospice. One resident on home health. Staff have CPR/First Aid training. Deficiencies were cited on this day. LPA requested that updated forms be sent in to update their facility file. Please provide LIC500, and a copy of current Liability Insurance by April 10, 2026. An exit interview was conducted with Administrator, and a copy of this report along with appeal rights was provided.
