Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed a chefs knife, tools and sharp cooking utensils in an unsecured drawer next to the stove, disinfecting/cleaning supplies under the resident bathroom sink and cleaning supplies unsecured in the staff room.

Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. R1's Physician Report (PR) or Service Plan (SP) do not accurately address Diabetes with Insulin, Oxygen Or Pure Wick Catheter Management. R2's PR or SP have not been updated since 2024. R2 has MCI, Diabetes with Insulin and Oxygen use.

§1569.312 Basic services requirementsEvery facility required to be licensed under this chapter shall provide at least the following basic services:(a) Care and supervision as defined in Section 1569.2.*This requirement was not met as evidenced by: Licensee did not ensure care and supervision was provided to R1. On 4/5/24, R1 left the facility alone without staff knowing. R1 sustained injury and required hospitailzation.This poses an immediate health, safety or personal rights risk to persons in care.

Complete admission suitability appraisal

Based on interview and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. The facility did not assess R3 prior to return from a hospital stay resulting in R3 returning with a cathater. Facility was not aware of the change of condition and cathater.

R1's service plan does not address Diabetes Management, staff assistance or Stage 2 wound care, R2 was assisted with Humalog incections of the wrong dose, service plan does not adress Oxygen use or Diabetic management

Based on interview and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. R1 and R3 are currently receiving services from Home Health Agencies. The facility does not have a written agreement or communication log of services being provided for either.

Store centrally held medications in locked secure place

87465 Incidental Medical and Dental Care (h)The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication.This requirement was not met as evidenced by: Licensee did not ensure that that all medications were centrally stored and locked, inaccessible to residents. LPA observed over the counter (otc) medications on the dresser and bedside table of R1.This poses an immediate health, safety or presonal rights risk to persons in care.

(1) Storage areas for poisons, and firearms and other dangerous weapons shall be locked.This requirement was not met as evidenced by: Licensee did not ensure that disinfectants and cleaning solutions were inaccessible to residents and locked. LPA observed cleaning solutions were stored in the laundry room cabinet and closet as well as in a bathroom under sinks.This poses an immediate health, safety or personal rights risk to persons in care.

Immediate disposal of contaminated food

87555 General Food Service Requirements (b) The following food service requirements shall apply: (28) All food shall be protected against contamination. Contaminated food shall be discarded immediately upon discovery.This requirement was not met as evidenced by: Licensee did not ensure that food was protected against contamination. Spoiled vegetables were found in the refrigerator.This poses a potential health & safety risk to persons in care.
