Inspector’s narrative

What the inspector wrote

On 12/16/2025 Licensing Program Analysts (LPAs) B. Miranda and J. Duarte arrived at the facility unannounced to conduct an annual inspection. LPAs were greeted by staff and allowed entrance into the facility. Licensee DeAngela Santos was contact and arrived shortly after. Facility is licensed for a capacity of 6 and currently has a census of 3. Facility has 4 bedrooms and 2 bathroom, 1 bedroom is used as an office. Upon entry LPAs observed residents engaging in activities. LPAs observed the facility to be clutter free, odor free, and exits to be obstruction free. Residents rooms are properly furnished with adequate lighting. Facility has 2 days supply of perishable food items and 7 days worth of non-perishable food items. Facility has medications, knives, cleaning supplies, and disinfectants locked and inaccessible to residents in care. Smoke and carbon monoxide detectors were tested and are in working order. LPAs observed all resident files which are completed and up to date. LPA reviewed a sample of staff files which are current and up to date. Training is current as well. Sample of resident medications was reviewed and no medication errors were found. No citations were issued. Exit interview was completed and a copy of this report LIC809 was printed and provided to Licensee DeAngela Santos.
