What the inspector wrote

On 05/28/2025, Licensing Program Analyst (LPA) Daiquiri Boyd arrived unannounced to conduct an annual inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Susan McCarty, who arrived shortly after. Facility email address and phone numbers were verified. LPA conducted a tour inside and outside of facility. Facility observed to be clean, odor free and at a comfortable temperature of 72 degrees. Common areas were furnished well with adequate seating and lighting available. Resident rooms appeared clean and had required furnishings. Resident bathrooms were properly equipped with securely fastened grab bars in toilet and shower areas, non-skid mats were observed. Hot water measured at 116.7 degrees F. Kitchen toured, appeared clean, food supply was checked. Exterior tour conducted, all exits open and free of obstructions. Swimming pool has a sturdy fence with a locked gate. Two storage sheds used for general storage, were checked and found to be locked with old furniture and garden supplies. All cleaning supplies are locked and secured under the kitchen sink. LPA observed the home to have the required postings located on the wall by the front door. LPA reviewed the homes Disaster and Emergency Plan and found it needed to be updated and was missing some details as to how the plan was to be executed; such as who is making calls to day programs and who is shutting off utilities if an evacuation is eminent. Facility sketch needs to be updated with evacuation details, meeting place, and exit routes, and utility shut off locations. Emergency food supply was observed, but found to be short in supply. Administrator is purchasing emergency food to be kept at facility. LPA observed that there was no variety in the stored food, having many cans of the same type. Fire extinguisher serviced on 06/04/2024. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Last fire drill conducted on 05/05/2025. LPA reviewed staff and client records. All staff and client files were found to be in good order. Medications reviewed and observed to have original labels and be administered as prescribed. (continued on next page) Exit interview conducted and a plan for Administrator to submit updated facility sketch and Disaster Plan was reviewed and developed with Administrator. A copy of this report was provided to Administrator, Susan McCarty whose signature on this form confirms receipt of this document. LPA is requesting the following documents be submitted to the Fresno CCL office by 06/15/2025: Current copy of Administrator Certificate, Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A), Surety Bond.
