What the inspector wrote

Based on observation and interviews, the preponderance of evidence standard has been met, therefore the allegation: Staff fraudulently signed facility MAR stating resident's medications were administered is found to be SUBSTANTIATED A deficiency is being cited in accordance to California Code of Regulations, Title 22, Division 6, see attached 9099D. An exit interview was conducted, and a Plan of Correction was reviewed and developed with Administrator. A copy of this report and appeal rights were discussed and provided to Administrator, Pamela Hamilton, whose signature on this form confirms receipt of this document

This requirement was not met as evidenced by: Based on interviews and record review, S1 made false/misleading statements when S1 initialed that medications were administered to residents when S1 did not administer medications. This poses an immediate health and safety risk to residents in care.
