On 01/28/2025, Licensing Program Analyst (LPA) M Vega arrived at the facility unannounced to conduct a Case Management visit. LPA was greeted by receptionist and stated the purpose of the visit. LPA met with Executive Director (ED) Eduardo Rangel. LPA conduced a follow-up on an incident report submitted to licensing 12/17/2024, in which it was reported that Client #1 was out of medication for 6 days. Incident was reported to ED and the client's Physician was notified. Client was placed on observation with no issues observed. Med Tech was was disciplined and retrained on medication administration and is being closely monitored for the foreseeable future with daily checks. ED stated that all Med Techs were retrained. No deficiencies were cited because immediate and appropriate action was taken.
