87465 Incidental Medical and Dental Care(c)(2) Once ordered by the physician the medication is given according to the physician's directions. This was not observed as evidenced by Facility failed to administer medications as prescribed. R1 was provided medications prescribed for R2. This poses an immediate health and safety risk to residents in care

Limit staff assistance with self-administered drugs

87465 Incidental Medical and Dental Care (a)(5): The licensee shall assist residents with self-administered medications as needed. This requirement was not met as evidenced by: Based on interviews and record review, Facility staff administered two 5mg tablets of Oxycodone to R1 instead of the ordered 5mg. This poses an immediate health and safety risk to persons in care.

87555 General Food Service Requirements (b)(5): Meals shall consist of an appropriate variety of foods and shall be planned with consideration for cultural and religious background and food habits of residents. This requirement was not met as evidenced by: Based on interviews, Licensee did not ensure residents received an appropriate variety of foods, which poses a potential health and safety risk to persons in care.

Diet quality and preparation in safe healthful manner

87555 General Food Service Requirements (a): The total daily diet shall be of the quality and in the quantity necessary to meet the needs of the residents...All food shall be selected, stored, prepared and served in a safe and healthful manner. This requirement was not met as evidenced by: Based on interveiws and records review, Licensee did not ensure that all food was prepared and served in a safe manner, when the temperature of meals where out of range on several days, which poses a potential health and safety risk to persons in care.

Keep prescriptions in original containers

87465 Incidental Medical and Dental Care:(5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. This requirement is not met as evidenced by: Based on interviews: Licensee did not ensure medications are stored in it's original container, when a tylenol was found in place of a narcotic, which poses a potential health and safety risk to residents in care.

87555 General Food Service Requriements: (3) Between-meal nourishment or snacks shall be made available for all residents... This requirement was not met as evidenced by: Based on interviews. Licensee did not ensure snacks were made available to residents in care when a staff member was found eating snacks belonging to residents in the facility bathroom, which poses a potential health and safety risk to persons in care.
