What the inspector wrote

On 03/16/26 Licensing program Analyst (LPA) M. Garza arrived at the facility for an unannounced complaint visit. LPA met with Executive Director, Sheree Addison, explained reason for visit and was permitted entry into the facility. LPA completed a health and safety check on residents in care. LPA observed residents in rooms, in common areas, and in kitchenettes getting ready for lunch. During visits for this complaint, LPA completed tours of the faciilty, completed interviews and requested documentation (medical assessments, staff schedules, needs and services plans, charting notes, staff and resident rosters). LPA observed meal service on 4 of 4 dates (8/6/25, 8/26/25, 11/24/25 and 2/24/26) and observed residents were not getting the required assistance in eating as needed. Meals were observed at bedside, in kitchenette refrigerators and in front of residents requiring assistance. Interviews disclosed that residents requiring assistance should be assisted prior to meal service beginning and "change of faces" should be provided to residents in an effort to get them to eat, if previously declined. Interviews conducted disclosed that this is not occurring. This allegation has met the preponderance of evidence standard per California Code of Regulations, Title 22. The allegation listed above is SUBSTANTIATED. Deficiency cited on 9099D. If not corrected, the deficiency poses a direct impact to residents in care. Exit interview completed with Executive Director (ED), Sheree. A plan of correction was developed by ED and reviewed by LPA. A copy of this report, deficiencies and appeal rights provided.

  • Personal assistance and care for required daily activities

    87464 Basic Services (f) Basic services shall at a minimum include: (4) Personal assistance and care as needed by the resident and as indicated in the pre-admission appraisal, with those activities of daily living such as dressing, eating, bathing and assistance with taking prescribed medications, as specified in Section 87608, Postural Supports. This requirement was not met as evidence by: interviews conducted and LPA observations. The licensee did not comply with the section cited above in that meals were observed at bedside, in kitchenette refrigerators and in front of residents requiring assistance. Interviews disclosed residents requiring assistance should be assisted prior to meal service but is not occurring. This poses a potential health safety and or personal rights risk to residents in care.
